4/3/2024 EnchantaVaughn Approved QMHPA arrived to the home of Chevelle. Upon arrival to the home setting, Chevelle appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. She verbalized to QMHPA that her day was going well. QMHPA informed Chevelle that this session was going to focus on anxiety and ways to cope with her depression. Chevelle did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Chevelle’s current thoughts, feelings and emotions. QMHPA encouraged Chevelle to discuss with her symptoms of depression and anger. QMHPA taught Chevelle a skills building activity to assist her with the use the skills taught to explore effective communications that is clear and concise. QMHPA used modeling and instructions to assist Chevelle. QMHPA educated Chevelle on how this skill will assist her with increasing her communications skills to meet her needs, develop friends and increase her social skills. QMHPA also informed her that this skill could also assist her with being able to build that bond with her family members and others that causes to her to become depressed and experience anxiety on a daily basis. QMHPA encouraged using calming techniques such as deep breathing or stepping away from moment to relax and calm down. Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Chevelle shared that she made several attempts to journal last night but couldn’t. Chevelle stated, “I can stay mad for a very long time”. Chevelle shared that she did understands the importance of listening and getting all the information before making any comments. Chevelle shared that she has tried of herself destructive behaviors. Chevelle shared that she was looking forward to having a better relationship with her family, but she has to be able to get over all the nasty things that some of her family members said to her and did that caused the tension between the two of them. Chevelle acknowledged that she still feels embarrassed because her behaviors can be out of control and inappropriate. Chevelle shared that she is willing to make behavioral changes as needed and be willing to take corrective measures to make the changes needed for her mental and physical health.
